Referral Process


EIM Referral Process  1


Step 1: M.S. Shook Student Health Services, Counseling & Psychological Services, or  Wellness and Prevention Services
At check in during the visit to the Health, Counseling Center, or with a Nutrition Specalist, students answer questions about their physical activity. 

PAV = physical activity is being taken as a vital signIn the visit and discussion, the Clinician determines the need and/or appropriateness of prescribing physical activity to improve health (mental or physical).

 

Step 2: Prescription and Referral

Students get a referral to a Personal Trainer and Behavioral Health Counselor to further discuss their current concerns as well as address goals as they relate to physical activity and getting physically active.  

 

Step 3: University Recreation and Behavioral Health Counselor

Students meet with an Exercise is Medicine Personal Training Coach to discuss individual goals and needs to becoming active.

 

  • Personal Trainers will provide education on EIM and why it is necessary for health and well-being.

  • Students will sign up here to begin a SMART START package with an EIM coach.

  • SMART START packages include:

    • 1 consultation session

    • 1 assessment session

    • 4 one-on-one 60 minute sessions 

Referral from the Health Center:

 

  • Students will meet with a Behavioral Health Counselor (BHC) to further discuss goal setting and behavior change. Part of the discussion will involve the benefits of physical activity and basic guidance regarding getting physically active.
  • BHC works with the student to develop a plan for how to get more active.

Step 4: Closing the Loop

Students will receive different options that are based on their goals and needs (additional sessions with BHC and Personal Training). Students may also follow up with where they received their EIM referral from.

Campus Ambassadors

EIM Campus Ambassadors are students, faculty, and staff who are dedicated to leading exercise is medicine as an initiative on App State's campus. Student-led clubs such as, EIM, Exercise Science, and Public Health serve as leaders on campus to promote physical activity and well-being.
